


Tournament Check-In Check List
All Leagues must have the following at check-ins:
Application and fees submitted to Tournament Director.
Laminated
Player Passes with correct player pictures and league stamp or signed by
registrar.
One
computer generated roster stamped or signed by registrar indicating that
everything is correct. Must include league name, team division, team number,
player names, birthdates, and player ID numbers.
One team roster for pictures (nicknames can be used). Must include league name,
team division, and player names.
Coach's Member Pass with picture, adequate coaching license level, and Risk
Management clearance.
Player Registration Forms also known as Medical Release Forms for all players
with signatures (forms with original signatures must be with coach at each
game).
Team contact sheet completed for each team & tournament. Must be turned in at
check-ins.
